Pharmacy Transfer Information
- Customer Name
- Date of Birth
- Your current pharmacy name and phone number
- A list of the medication you need to transfer
Carolina Apothecary will contact the pharmacy where your prescription is currently on file to get the information needed to fill your prescription. If the prescription has no remaining refills, we will contact the prescribing physician. Finally, you will receive notification when your prescription is ready for pickup.
